[Renaliste] Réaction à l’hémodialyse

Chers amis,
Nous avons un  patient de 77 ans avec insuffisance  rénale chronique qui a une réaction répétitive au cours de l’hémodialyse.  Voir en bas la description en Anglais par ma jeune partenaire, Julia
Il a eu un accident vasculaire cérébral (hémiparésie à  gauche) au mois de Juillet 2012 et est devenu urémique nécessitant l’initiation à l’hémodialyse. Apres 2 heures d’hémodialyse, il développe  cette réaction :
1.	Agitation/colère
2.	Tremblement des bras et jambes
3.	 Sibilance  audible avec hypoxie-réversible avec le traitement à nébuliseur à l’ Albuterol
4.	 En une occasion, il a même épisode temporaire de dysarthrie,  hémiparésie  à gauche, incontinence,  désorientation après la séance de l’hémodialyse.

Nous avons cru au syndrome de déséquilibre aigu et avons réduit le débit sanguin et de la solution de dialyse, utilisé le mannitol 
Nous avons changé de membrane de dialyseur, rincé le dialyseur avec 1 litre de solution physiologique
Nous avons stoppé le fer (sucrose) et l’Epogen

77 yo gentleman we have been caring for on our inpatient service with history of granulomatous disease. 

This week when I have been caring for him, 2 hours into treatment, he starts having the following symptoms:
1) Agitation/anger (in a usually nice gentleman)
2) Shaking chills
3) Audible wheezing with corresponding hypoxia down to SaO2 85-89% on 3L/min O2 via nasal cannula

Last time I dialyzed him he developed a CVA like syndrome after he returned to the floor with slurred speech, incontinent of urine and bowel, inappropriate answers to questions, flaccid left side. He had a right pontine infarct CVA at the begining of July - with left hemipareisis. Neurology did not feel that he had an actual CVA and also felt it was due to metabolic issues with dialysis. Slowing BFR 200/DFR 500 and using mannitol has not helped. Changing to Exeltra and flushing dialyzer with NS 1000 ml has not helped. I have not yet tried to start steroids/or benadryl.
